Insurance and Payment Policies

Perinatal Associates will file insurance forms for all services performed in our office or at the hospital. We only ask that patients bring either an insurance form or insurance card with them for their initial visit. All patients are required to pay 20% of charges at the time of service. This includes patients who have no insurance or whose insurance does not include maternity coverage. The only exception is for patients covered by an HMO who will be responsible for making their own co-payment. Payment schedules for any balance due must be arranged with our billing office.

We are participating providers wtih Presbyterian Health Plan, Presbyterian Salud, Santa Fe Healthcare, Molina Salud, United Healthcare, Admar, Blue Cross Blue Shield of NM, Tricare, Medicaid and Indian Health Service, Principal, Mutual of Omaha, Great West, Lovelace Health Plan, Lovelace Salud, and Cigna Health Plan.

We accept MasterCard, Visa, Discover and ATM. Please call our billing office at 764-0148 with any questions regarding insurance, referrals or payment.
